Social Service Department

The Social Service department assists residents and their families with orientation and adjustment, counseling and problem solving. Social Services assists in the Coordination of Care Conferences to regularly review the plan of care with each resident, their family and the staff team.

Discharge planning is provided as residents improve and return to their home environment
