Intermediate and Skilled Nursing Care

Intermediate and Skilled Nursing Care is available for those who require daily assistance with medical and personal needs.  Our nurturing and dedicated staff is comprised of qualified professionals, including registered and licensed practical nurses, therapists, activities and social service personnel, who combine up-to-date skills with loving, faith-based service. Care Plans are designed by multi-disciplinary staff members, using clinical recommendations and in collaboration with family members to ensure superior quality and consistency.

Hearthstone Manor is Medicare Certified and provides rehabilitative therapy by licensed physical, occupational and speech therapist that promote recovery from an illness or injury and return to the home environment.

Rehabilitative nursing care is available to sustain gains made with the licensed therapist.  Our goal is to promote the highest level of independence in each individual.

Regular visits are made by primary care physicians, a dentist, podiatrist, optometrist and other medical professionals.

Rooms on the Nursing Care floor are typically semi-private.  Residents in each room have a bedside nightstand, and share a wardrobe with hanging space above and drawers below.  Rooms have hospital style beds for maximum comfort and flexibility.  All bed and bath linens are provided.  Families are encouraged to bring decorative accessories from the home environment to promote welcoming surroundings and familiarity.
